Patient Information for Chronic Health Conditions

Our goal at Integrity Wellness is to provide for you the highest level of individualized care to help you reach your optimal health.

Please read all of the information contained in the documents below in detail. The information required may seem extensive and we realize that not all questions apply to all of our patients. Our goal is to be very through leaving no stone unturned. Many of our patients have had health conditions that have lasted for many years and have seen a multitude of physician prior to coming to Integrity Wellness. Our goal is to look at the whole person physically, chemically, and emotionally to effectively treat the patients and make appropriate recommendations, and follow them through their roadmap to healing. Thank you for your time completing this important information. If the consultation is for your child (under age 18) please use the forms under Pediatrics. All other patients use the general patient forms.

The amount of time patients are seen for the initial consultation and examination varies dependent on their condition(s). Generally for acute injuries and conditions that are less than 3 months old we schedule 45 minutes, and 80 minutes for patients who have chronic issues.

All forms must be complete and mailed, emailed, faxed or dropped off at our office so they arrive at least 3 days prior to your appointment. This allows Dr. Petro to review your information prior to the consultation and examination, making the visit more efficient and enhancing the quality of your care.

All necessary patient forms are located on this website under New Patient Information. If you have any problems downloading any of the forms please call our office immediately.

Your medical records can only be released with your authorization. Dr. Petro will need to have copies of your labs and diagnostic studies for at least the past 3 years. A medical records release form is enclosed in the new patient forms. You are responsible for obtaining previous medical records from other physicians or health care providers. Your records should be mailed to the address below or faxed to (419) 424-1188. If you have difficulty acquiring you records please call our office immediately. These records are very important and are necessary to receive prior to your consultation and examination.
