Patient Information for Pediatric Chronic Health Conditions

Our goal at Integrity Wellness is to provide for you the highest level of individualized care for your child and to help them reach their optimal health.

Please read carefully all of the information contained in the download box to the right. The information required may seem extensive and we realize that not all questions apply to all of our patients. Babies only a few months old will not have as much history as a teenager. Our goal is to be very thorough, leaving no stone unturned. Many children who come to see Dr. Petro have had health conditions that have lasted for many months to years and have seen a multitude of physician prior to coming to Integrity Wellness. Our goal is to look at the whole child physically, chemically, and emotionally. Our goal is to help your child travel successfully on their roadmap to healing. Children who have chronic health problems are scheduled for a 60-80 minute initial consultation and examination.

All forms must be complete and mailed, emailed, faxed or dropped off at our office so they arrive at least 3 days prior to your child’s appointment. This allows Dr. Petro time to review your information prior to the consultation and examination making the visit more efficient and enhance the quality of your child’s care.

Your child’s medical records can only be released with your authorization. Dr. Petro will need to have copies of their labs and diagnostic studies (if your child has had any) for at least the past 2 years.  A medical records release form is included in the Pediatric New Patient forms. You are responsible for obtaining previous medical records from other physicians or health care providers. Your records should be mailed to the address below or faxed to (419) 424-1188. If you have difficulty acquiring you records, please call our office immediately. The labs and tests are one of the most important aspects of your child’s assessment. Many times children have not had any diagnostic tests or labs, and if this is the case there is no need to use the records release form.
